Care Navigation

The Care Navigation team of Powers Health Partners provides outpatient care management services to all members who may benefit from assistance navigating through the healthcare system, from health promotion to complex medical management. One on one support by skilled nursing professionals help to coordinate care of members by identifying gaps in care, encouraging wellness visits and assisting those identified as high risk. Using sophisticated risk stratification tools our care management team is dedicated to providing a continuum of care that not only improves patient care but demonstrates value to our providers.
