The Community Health Needs Assessment (CHNA) examines the health status, behaviors and most pressing health needs of the cities and towns in Northwest Indiana served by the four hospitals of Powers Health.

Insights gained through this survey process inform the hospital’s plans to address health disparities and bring about improvements in the health status of the community as outlined in its Implementation Plan.

A CHNA is conducted every three years according to regulations pursuant to Section 501(r)(3) of the Internal Revenue Service Code. The report describes incremental changes toward fulfilling the goal of better health and an increased lifespan for area residents.
