Our oncology nurse navigators provide patient-centered care from the time of diagnosis through each person’s cancer journey into survivorship. These nurse navigators work with patients, families and caregivers and help to ensure that all key elements are provided of high quality oncology care. Our priority is to streamline each patient’s oncologic care and remove barriers to treatment which will help reduce delays in accessing our multidisciplinary oncology care services.

The breast cancer survivorship program at Community Hospital, St. Catherine Hospital and St. Mary Medical Center is supported by the services of our breast health navigators. When survivors finish their active treatment for breast cancer, they have the option to meet with one of our nurse navigators to review/receive a comprehensive survivorship care plan. This plan is a detailed summary of all treatment and recommendations for follow-up care. The nurse navigator helps cancer survivors transition into recovery, focusing on how to achieve the highest quality of health and life following cancer treatment. With coordination from a medical oncologist, a plan is put into place for each patient to ensure that they have the resources and knowledge of what is needed for their follow-up care. The nurse navigator can obtain referrals and answer questions patients may have about their ongoing care plan. This is a complimentary confidential service provided by Community Hospital, St. Catherine Hospital and St. Mary Medical Center.

Being diagnosed with breast cancer can be a challenging time and Powers Health recognizes the complexity of needs that arise with a diagnosis. Our breast health navigators are here to help make this experience a little easier for patients, families and caregivers by providing ongoing education, guidance and emotional support throughout the cancer care continuum. Breast health nurse navigators empower patients to work as a partner with their healthcare team and serve as patient advocates which promotes well-coordinated, timely care. Our nurse navigators are with you from the time of diagnosis to recovery and beyond. At any time, patients and their family members can access our nurse navigation program which is a free, confidential service of the Women’s Services.

Lung cancer is the single leading cause of death in the United States. Research shows that having an annual lung cancer screening for those qualified individuals can help find a lung cancer at an early stage and prior to the onset of symptoms.  Early diagnosis of lung cancer is critical as there are often more treatment options available and the survival rates are greatly improved. Our certified oncology nurse navigator serves as coordinator for our low-dose lung cancer screening program and helps guide patients through the screening, enrollment and appointment process. Our certified oncology nurse navigator is the single point contact for patients, families, caregivers and referring providers, which helps reduce barriers to care that often can be encountered in the medical setting. Our oncology nurse navigator also is a valuable resource to patients diagnosed with lung cancer and is available to help guide and support patients and their loved ones through the cancer care continuum. Our oncology nurse navigator helps educate patients on how to obtain the highest possible quality of health prior to and following a diagnosis of lung cancer. This is a free confidential service.

Our lung health nurse navigators serve as a resource to patients, families and caregivers who have been diagnosed with lung cancer. Often, the first point of contact is made at time of lung cancer screening or shortly after a diagnosis of lung cancer is received. The lung health nurse navigator helps guide and support patients and loved ones throughout the cancer care process, providing essential information to ensure well-informed decision-making, treatment planning and timely coordination of care. An important part of this free service is helping to educate and support patients regarding how to maintain good health and optimum quality of life both during and after treatment for their lung cancer. Once treatment is completed, patients have the option to receive a customized survivorship care plan, which is a comprehensive treatment summary of their cancer care and recommended follow-up with their multidisciplinary care team. Additionally, the lung health nurse navigator facilitates smoking cessation counseling. This is a free confidential service.
