We are committed to providing patients and their loved ones the best compassionate care, experience and treatment in everything we do. 

Admission Process

Whether your hospital stay was planned or the result of an emergency, you or your healthcare representative will be asked to sign a number of health care consent forms to provide the best care and service.

The forms include:

  • Consent to treat and/or perform surgery
  • Assignment of benefits
  • Release of medical information
  • Responsibility for payment and personal belongings

Information obtained by hospital admissions personnel is kept in strictest confidence. If you have questions about your treatment, please direct them to your doctor.

Insurance companies typically require that you notify them about a hospital stay. Contact your provider before you are admitted with any questions about coverage.

  • Photo ID, a driver’s license or state ID
  • Insurance card
  • Names and phone numbers of someone to contact in an emergency
  • Copy of your Advance Directive, including a Living Will and/or Durable Power of Attorney, if you have those documents
  • List of medications you take on a regular basis. Do not bring medications from home, unless your doctor writes an order to do so.
  • Pajamas, robe and slippers for comfort
  • Toiletries
  • Dentures, hearing aids, contact lenses/eyeglasses in labeled containers
  • Small amounts of cash to buy a snack, reading material or convenience items
  • Radios
  • Cigarettes, cigars, pipes, smokeless tobacco/nicotine products and smoking products
  • Weapons

The hospitals of Powers Health are tobacco-free facilities. Smoking is not permitted by anyone in any of the hospitals’ buildings, vehicles, parking lots, grounds or off-site facilities. Tobacco use includes cigarettes, cigars, pipes, smokeless tobacco products and devices or materials emitting or exhaling smoke or other substances.

Ask a nurse about Nicotine Replacement Therapy, if assistance is needed.

You will receive an activation code to MyChart, a patient portal for health information. MyChart offers personalized and secure online access to your health information communication opportunities with your health care providers.

Family Birthing Services patients can download a MyChart Bedside app to manage and view hospital stay information on their touchscreen tablet.
