Navigation Care Services

Our navigators provide individualized, proactive guidance to patients, families and caregivers throughout the cancer care journey. Their focus is to streamline each patient’s care and remove or minimize any barriers for to ensure the best possible health outcomes. Additional benefits of having a navigator include:

• Provide continued social and emotional support to the patient, family and caregivers, beginning at time of diagnosis
• Empower each patient to communicate their preferences and priorities for treatment to their healthcare team, as well as loved ones
• Promote patient autonomy and well-informed medical decisions by providing evidence-based information regarding available treatments and clinical trials
• Identify and address the needs and challenges that each patient may encounter throughout their cancer care continuum
• Serve as a liaison between each patient and their multidisciplinary healthcare team which improves timeliness of care and positive health outcomes
• Assist with referrals to supportive care services as needed to promote the highest quality of life before, during and after completion of a comprehensive cancer treatment
