Our diabetes self-management education and support programs focus on the self-care behaviors identified by the American Association of Diabetes Care and Education Specialists:

  • Reducing Risks

We work as a team to address each person's unique concerns, combining the expertise of the patient's healthcare provider, certified exercise physiologists, pharmacists, registered dietitians, registered nurses, and our diabetes care and education specialists. Education and support are offered for both outpatient and inpatient needs.

Education classes include:

Diabetes self-management education and support is recognized as an important component of patient care and recovery in the hospital setting. Our goal is to prepare the inpatient to perform necessary survival skills by the time of discharge.

We conduct additional diabetes programs for the community, such as Health Fair education, outreach to schools and diabetes awareness campaigns.

In these individual sessions, a registered nurse and registered dietitian review your diabetes diagnosis and treatment, meal planning, blood glucose monitoring, exercise, stress management and medication therapy.

We offer individual training for injectable medications, self blood-glucose monitoring, or for anyone who has special requirements. The classes at Community Hospital and St. Catherine Hospital are recognized by the American Diabetes Association for Quality Self-Management Education.

A registered dietitian can help you develop a personalized meal plan on an individual or group basis to help you achieve goals for weight management, blood-glucose levels and a healthier lifestyle.
