The Medication Management and Anticoagulation Clinics at Powers Health are staffed with experienced pharmacists who provide therapy management to patients taking blood thinning medications.
Patients at risk for blood clots and prescribed anticoagulation (blood thinning) therapy by their physician can benefit from services provided by the Medication Management and Anticoagulation Clinics at Powers Health. Our pharmacists who work with patients and physicians to ensure safe and successful management of anticoagulation therapy through education, comprehensive patient counseling, medication monitoring, and coordination of patient care plans. Providers may refer patients at any stage of anticoagulation therapy to the clinic.
